Community Health Partnerships

Introduction

Community Health Partnerships (CHPs') were formed to be part of a decentralised but integrated health and social care system to manage a wide range of services and have a pivotal role in delivering health improvement for their local communities.

The CHPs' will be responsible for reducing waiting times for diagnosis, treatment and care. Decrease inappropriate hospital visits. Reduced emergency admissions and delayed discharges. Improved provision of chronic disease management. Reductions in premature and preventable deaths. Improved access to services and reduced health inequalities.

The committees have to have a number of specified representatives by statute. These include a Voluntary Sector Member, a Carers Representative and members from the Public Partnership Forum. To date only the voluntary sector role has been filled.

The City of Edinburgh has 2 CHP committees (North and South). Both committees have met and have discussed an number of topics with scooping presentations from the voluntary sector on issues around Health Inequalities, Mental Health and Older Peoples Issues.
