Town wide social referral & community engagement system, 3 year study.
Across the whole of Somerset, emergency hospital admissions rose by 29% during the three years of the study, in Frome they fell by 17%.

Transformational Change, Glasgow City Health and Social Care Partnership (GCHSCP)
Children and Young People’s transformational change programme 2018-21: GCHSCP have begun to move expenditure from high cost residential care to delivering preventative & early intervention support work with families, to ensure that, where necessary, more young people are sustained at home for longer. BLF have pump-primed with a Systems Change Fund. Budget to third/community sector is being increased, not cut.
